Cyber Security Incident Report
This report form is used to report suspected or confirmed cyber security, information security, IT, or data protection incidents affecting EQUA systems, services, customers, employees, or information assets.
Examples include:
- Service outages or disruptions
- Unauthorized access or suspicious activity
- Malware, phishing, or compromised accounts
- Data loss, leakage, or unavailable data
- Security weaknesses or vulnerabilities
- Supplier or third-party security incidents
Please provide as much relevant information as possible, even if all details are not yet known. Early reporting is important and helps us assess, contain, and manage potential incidents in a timely manner.
All reports are reviewed by the responsible security and IT personnel. Depending on the severity and scope of the incident, the matter may be escalated internally and, where required, handled in accordance with applicable regulations such as NIS2 and GDPR.
After submission:
- You will normally receive a confirmation that the report has been received.
- Critical incidents are reviewed and escalated as soon as possible.
- Non-critical incidents are normally reviewed during business hours.
- Additional information or follow-up questions may be requested.
Note! Do not use this form for emergencies requiring immediate operational action. In urgent situations, directly contact the IT or security responsible person by phone or other established emergency communication channels.
